cross over fee help please

I want to cross over a card from sgc. I have never does this. I see where to post a min grade that I will accept, but how much does this service cost? Also can I send this right with my current order ready to grade and include it on the shipping back to me so I don't have to pay shipping on it separately?

    Crossovers are the same cost as regular grading, and they need to be on a seperate submission form from the raw cards that you send in the same package.
