Not an error. Looks like someone has milled out the obverse to make a magicians coin.
Unless you were there at the time you might never know exactly how it happened.
It is just a damaged nickel now.
It looks like one of those trick "squirting" coins. Put in water to fill it, squeeze the brass piece and water squirts out a hole on the other side. I think I see the hole in the O in OF.

Dang, you're good. That's exactly what it is. I recall seeing ads for those years ago.
Submerge it in water, press on the brass back a couple times to draw water into it, then show it to soneone as you press the back and it squirts water at them.
