2013 D nickel - error or just PMD with Heat Enhancement

I found this 2013 D (maybe a 2012 D) that looks like heat enhanced PMD. If it is an error how could it be done at the mint? Your thoughts and comments are welcomed. Thanks in advance. :)

    Not an error of any kind. That would be impossible to happen
    with the coin in the striking chamber. It doesn’t really matter
    how it happened, damage is damage.

    @Greenstang Thanks for the comment. It looks to me like someone used a torch to what would happen to Jefferson.
