Two different dies attributed to the same holder?

Given that my specialty is Ikes, and that the finer points of other series are not as well known to me, I querry the group:

Background
In the Ike series, two different peg leg dies (see below) are typically (routinely) assigned to FS-401 attribution. The two obverse dies are identifiably different - the second of the two obverse dies even has two reverse die marriages). I'm not speaking of wrongly-attributed coins ending up in an attributed holder. I'm speaking of two different dies ending up routinely attributed into the same attribution holder.

Are there examples of coins from two different dies ending up routinely attributed into the same attribution holder in any other series?

    This is a question for PCGS and yes, PCGS routinely attributed both dies to the same number regardless of what the CPG Illustrates. I do not know why, I just know that they see "ALL" PegLegs the same way.

    This will be a problem in the future but it's really up to the IKEGroup to bring it to their attention. Specifically, Mike Faraone, the chief attributer.
    I decided to change calling the bathroom the John and renamed it the Jim. I feel so much better saying I went to the Jim this morning.

    Lee - I'll follow-up, as you suggest. However, I'm still interested in finding out if this happens in other series. (again, not attribution errors, but actual routine attribution of coins from two separate dies to one attribution number / holder)

    Are there examples of coins from two different dies ending up routinely attributed into the same attribution holder in any other series?
