Error guys: what is this??

Handed to me by a friend who has had this hidden away for many years. He saved it because it was so thin. The diameter is also a little small.
Three things come to mind for me when looking at it:
1) wrong planchet? (wrong)
2) split after strike? (wrong)
3) grease strike through on the rev? (wrong)
Some sort of odd combo of problems? (or something totally different and not a real error at that)
