Perhaps the back of the card has a crease? That has to be the case.

Any chance the checklist on back was checked in pencil and erased or something like that, or does that automatically result in a MK qualifier?

PSA also down grades a card if a crease or wrinkle has been flattened out by 2 grades. perhaps this is the case here.
Good for you.

I think I might have found it ( left of the 1 and goes through the o in Chicago) not easy to find but its shows better in the back of the card!
