That looks neat, but it seems too easy for another country to create errors. I would not consider this rare, but a “mint-on-demand” type of thing.

I'm not sure but I think we do produce (mint) coins for other countries so something like this, though I assume very rare, could and did happen.

Another planchet error, one that is highly sought by collectors, occurs when a planchet intended for a foreign nation’s coinage is struck by dies for a United States coin. This type of error was more common before 1984, when the U. S. Mint cut back its production of coins for other countries.

This error was struck at the U.S. Mint. Here's the text under the photo:

"This is a spectacular double denomination involving coinage from two countries. The U.S. Mint struck coins for the Philippines in the 1970's including the Philippine 1 Piso coin. It is a large coin and there are few major mint errors known on this denomination. Somehow a struck U.S. Quarter was overstruck off-center by the Philippine 1 Piso dies. It is a spectacular and dramatic major mint error and it is extremely rare".

I'll bet it was struck in San Francisco. There were a LOT of "helped" errors that came out of the San Francisco mint in the late 60's and early 70's. On of my favorites is a 1970-S proof quarter struck on a 1900 Barber quarter.
