What is it?

It is 48 mm in diameter and 6 mm at it's thickness. It is a George III Death Medal made in the early 1800s. It came from Mathew Boulton's personal collection. These were stored in brass shells. This one only came with half the shell.

Below is another example showing the reverse, both shells and a piece of paper it was stored in with James Watt Jr's own hand writing on it. ( Mathew Boulton's partner )

That surely is what I'd call high relief and I was surprised at the quality and preservation of a coin made in the early 1800's.

These are very interesting coins. Are the Boulton and Watt fellows related to steam engines or was that later in the 1840's.

Awesome medal, I liked the original images you posted in the World Forum even more. They showed the quality/depth of the copper more than your mirrored pic. That wreath design is something US designers can learn from, I love it.
