Icons, How do you customize?

I've searched and searched, clueless as to how you customize your Author Icon...Help
I have my own photo I would like to use, not one of the listed ones.

You can upload on the 15th of each month.

Beware Carol said it will be this Friday the 14th so mark your calender's. I hope my new coin comes in in time to make a new icon.
