I am not sure where you are sending these, and do not see the coins in person, but I would suggest saving your money and forget about slabbing the last 3, and spend it on some more coins, or a weekend trip to a coin show and look at dealer slabs.
That nickel looks like a simple case of a nickel that has been buried for a while. What is its weight? How do you know its composition, or is that a guess?
On this board, most people would say (in differing orders): PCGS, NGC, and ANACS as being the only slabs worth dealing with. Some would say never slab. Your choice, but no one regards NTC, ACG, and the like with any respect on this board.
Of the big three I think you would be beter off to send them to ANACS. I think they have more experience with errors and they will do them for the standard submission fee while I believe with PCGS you have to submit them under the error class at $30. If you send them under the regular submission rates they will either bill you for the difference or send them back as bodybagged as not a proper submission. And you would still be out the fee.

Good point, what others do members recommend?
I got the coins and the form ready to go to ANACS.
I saw NTC on Ebay quite often, why I originally went with them.
