It appears to be a sample made by ACG to sell rights to their holder? I’ve read a few forum posts describing similar slabs but none of the photos are available and nothing explicitly says anything about “your name or co. On two lines” (that I can find).

I collect older NGC samples so I’m not new to the game but I don’t venture into the non PCGS/NGC samples.
