    Go the PCGS home page, click on Services, then Fee Schedule, then scroll down and you will see a link to download a quarterly special submission form.

    ajaan, that is correct. The link should be taking you to a Secure Plus submission since the quarterly freebie is for one free Secure Plus submission of a world coin. Thanks!
