I guess it depends on whether you're judging the coin by the "old" standards, or the "new" standards. My suggestion is to study online grading resources and talk to a lot of older, experienced, reputable coin dealers, about grading standards (at coin shows for instance), and examine/compare as many PCGS and NGC graded MS 65 coins in person/in hand as possible, to get a good feel for what's 65 and what isn't.

I had been under the assumption (based on the touts of countless dealers) that since a coin is in an OGH or older NGC holder, that it is an excellent candidate for upgrade. Evidently this may not actually be the case?
Grading is subjective and this is also why people get coinfused. And on top of that, time changes everything, as many have witnessed. The grade should not change but as people subject coins to different environments, it can profoundly affect a coin. And that's why "eye appeal" is the all important factor in the grading game and it's also why color/hazing/spotting can be a detriment, over time.
