How do you go about submitting a coin for review under the PCGS Grading guarantee?

I was going through some of my Ike dupes the other day and notice spots on a MS68 73-S. The spots are new and it sure wouldn't have graded 68 with them so how do I go about sending it in under the grading guarantee?

On the submission form under other, just write in spot review and only write in the amount for shipping.

On a piece of paper write down your concerns and the general location of the spots.
