    Pretty sure that's an O-115

    As for a grade, the lower they go, the harder time I have in being in the ballpark, but when that much of the rim is gone, maybe G4 at best, but I'd probably call it an AG3

    It looks surprisingly good for that much wear

    If you are buying it will be a G6 based on the obverse. If you are selling it will be an AG3 based on the reverse.
